Fill out your Health Risk
Questionnaire and send it in!
Picture this:
You’re walking down the street and on the corner is someone in a booth handing
out cash. “Free money to anyone who walks by!” he says.
My assumption
is that you’d take the cash, wouldn’t you?
Chances are,
if someone was doing that, there would be police on the street directing traffic
and taking care of crowd control.
Then why
haven’t most of you sent in your Health Risk Questionnaire (HRQ)? It is free
cash.
By not
filling out the questionnaire and failing to send it in, you are turning down
money that is yours!
I say most of
you, because as I write this column, only 18 percent of the active membership of
this local union eligible for the Indemnity PPO Medical Plan with the UFCW Food
Trust Fund has turned in their HRQ. That’s right, nearly 80 percent of those
eligible to qualify for an enhancement of their medical benefits have flat out
ignored the opportunity!
Unbelievable! It would only take a little effort to bump up the value of what
you have so rightly earned.
In an
article last month, we advised you that in your mailbox you will find an
important tool that can help you improve your quality of life — and get paid for
it! It’s called the Health Risk Questionnaire.
You have
received mail from the Trust Fund letting you know that the HRQ would be
arriving. You also received a DVD explaining the HRQ and how it benefits your
life.
With the HRQ
you can make a comprehensive assessment of your overall health and, in the
process, identify health risks that you should address.
After you
have completed the HRQ and sent it in, you will receive a personalized wellness
plan, as well as information tailored to your situation that will guide you in
making healthy changes for life.
The HRQ is
voluntary, completely confidential and — as part of the New Indemnity Medical
Plan — there is no cost to you.
In fact, you will receive an extra cash contribution to your
Health Reimbursement Account if you complete and return your questionnaire by
the deadline. |
